\r\n\r\n The following Care and support services are provided by The\r\n Agency: Alzheimer's, Aids/HIV Cancer care, Palliative care,\r\n Cerebral Palsy, Motor neurone disease, Multiple sclerosis,\r\n Parkinson's disease, Stroke and Day care. Service User care\r\n plans are reviewed on an individual basis, according to assessed\r\n need, but at least every month.\r\n
\r\n\r\n During this journey we work closely with every service user to\r\n develop their life skills, taking controlled risks, becoming\r\n independent, gaining confidence.\r\n

We provide care to meet a\r\n comprehensive range of client needs in order to help adults stay\r\n independent and live in their own homes or supported living\r\n schemes. We aim to involve Service Users and their loved ones in\r\n the assessment and implementation of their care needs ensuring\r\n that the service is provided and run in the best interest of\r\n Service Users.\r\n

\r\n\r\n Just because you may have a physical disability does not\r\n mean you can't enjoy a full and active life living in your\r\n own home. Our professional carers will listen to your\r\n needs and put together a bespoke support package to meet\r\n your requirements. This can range from once a week visits\r\n to daily visits or even live in care. We can help you with\r\n a wide range of tasks including domestic duties around the\r\n home, personal care and washing, social appointments and\r\n hobbies. Indeed, our support packages are aimed at helping\r\n you stay independent in the comfort of your own home,\r\n allowing you to maintain your everyday routine.\r\n

\r\n\r\n Privacy\r\n The right of individuals to be left alone or undisturbed\r\n and free from intrusion into their affairs. This is\r\n taken into account in the formulation of Care Plans, and\r\n will only be overridden in exceptional circumstances,\r\n and with the knowledge of the service user or their\r\n advocate.\r\n
\r\n\r\n Confidentiality\r\n Service user confidentiality is, wherever possible,\r\n maintained. On occasion, it will be necessary, for the\r\n benefit of the service user or others to share personal\r\n information with either other professionals or\r\n organisations. Where possible, service users are\r\n consulted and their views taken into account.\r\n
\r\n\r\n Dignity\r\n All individuals, whatever their circumstances, have the\r\n right to be treated with dignity and respect.\r\n
\r\n\r\n Choice\r\n Services are designed to be accessible and flexible,\r\n promoting ordinary lifestyles and based upon client's\r\n own choices. Individual choice is promoted, within the\r\n limits imposed by service constraints.\r\n

\r\n\r\n Responsibilities\r\n Clients are expected to accept appropriate\r\n responsibilities, taking into account their particular\r\n abilities and circumstances. These may include having\r\n due regard for others, for property or for participation\r\n in Care Plans.\r\n
\r\n\r\n Family Support\r\n The value of the contribution of client's family support\r\n is fully recognised. The needs of the client we are\r\n providing, or intending to provide, regular or\r\n substantial care is, where requested, assessed\r\n independently of the needs of the client. The outcome of\r\n the assessment is taken into account when agreeing Care\r\n Plans.\r\n
